I'm looking at league average and my staff salary is higher by approx 50K than others.
Which one should I fire and downgrade?

Both the doctor and trainer are hideously overpriced.

well, their salary increases weekly.. am I suppose to fire them and replace them with cheaper alternatives on a regular basis?

Yes. I replace all my staff approximately once per season, although some guys I've kept a little longer.

Also, when shopping for new staff, it is a good idea to try to buy ones with low initial weekly salaries. This helps a lot in controlling your staff costs.

Indeed.

I was able to get a level 6 doctor with a $19K salary for $2K
