Since fouls lead to injury, I would like automatic suspensions for a league game for each foul that leads to an injury. Flagrants also should lead to automatic suspensions after the second or third in a season irrespective of injuries.

Otherwise, please let us target opponents players with defensive intensity so that we can exact retribution in the following game.

Thank you.

I agree, for the sake of realism

Why would I want to help the next opponent of the "offender" team?

It's a penalty like any other. It works like that in any league in any team sport in the world, as it has nothing to do with team responsibility but with individual responsibility.

I'd like the option to target a player or a team with more and harder fouling. That is extremely realistic and discounts the fact that in the NBA there is intentional fouling (hack-a) on poor FT shooters, that everywhere in the world there are 'enforcers', players who are there to prevent you to break or embarass their teammates, and fouls are part of the tactics to break down the opponent's rhythm. Of course whoever takes out an opponent must get a suspensions.

This is actually way more realistic than, say, the way injuries are currently dealt with.

Targeting poor FT-shooters should be an option indeed and would add some realism. I would not like to have designated targets that should be injured. Enforcers are long gone.

You could get insurance against your poor FT shooters being injured by constant fouling...

Bogut, Perkins, Barnes, Haslem, Adams sure there are less than in the past, but there are still some.
