48+ means that the player has played at least 48 minutes and 0 seconds week, while 48 means that the player has played between 47 minutes and 30 seconds to 47 minutes and 59 seconds that week.

It's not full training but close. How close depends on how many seconds a player missed. If you go on the game log in the player page you can see decimals of the minutes played, that's the best you'll get from BB pages.
