Even though you're close to the paint, you might want to shoot very high to avoid a block attempt I guess!

For number 2 it is possible that the defender blocked the dunk from under/through the rim, which will be a goaltend.

OK, looks like #2 and #3 are essentially possible so I'm just going to comment on #1, which is not that unusual, but in order to see the error in action, you should specify the exact time of the game when it happened, so I can see if it can be reproduced. If not, it's probably just a glitch in data flow from the internet, therefore out of our control.
