Nachtmahr didn't quite get there, his player Haek had no 20 in those skills (it was 18 if memory serves me right, which is a more reasonable target). And he also came short of his planned target, so I'm pretty positive 20 OD/PA/ID/SB may be achievable only with good starting skills, perfect height and sacrificing everything else.

I would give up OD and train RB instead. Get mediocre to decent JS/JR and just get the player to play PG and defend inside. Something like: 11/11/13 9/9/20 9/19/19/16. This alone is a very challenging target, but at least the inside skills will help each other with elastics and the only thing you will really train very much against elastics is OD. This player can hit the occasional 3pt shot and layup on a mediocre, but not horrific FG% especially if he is able to get to the line despite the lack of DR. He would mostly pass the ball and lock down big men.

Haek had both D at 17-18 and SB at 19. For a player like this you should aim for really low Driving (only Handling ideally) provided you just want a wall. Players like these are useful on NTs. They should also aim for a +12RB since 90% of the time that's the position they'll guard.

I've trained player who have ID20, SB20 and OD18. He also have nice passing skill...

These players are all black holes on offense. The only way to limit the damage is to make them into a PG and hope they don't shoot (so I would really go for someone with starting 1 DR and never ever train DR or HA). However training a 140 TSP player against elastics is pure fantasy.

I trained a big man with almost 130 and 66 inside skills and with 12 IS and 13 PA he was a huge problem on offense. I've seen players with 19/21 ID/SB in the EBBL and they did not stop top big men. In fact too high OD may even force more inside shots which have a more efficient baseline even against elite defense.
