Believe me, I am really annoyed when a trainee gets 46-47 mins with no apparent reason and I have to throw away 1-2-3% of his training for the week (for which I'm paying 15-20k/week so to speak).

My opinion on the matter however is that the game is better this way. There have to be unexpected situations. We should be pressed to make hard choices between the lesser of two evils, otherwise the game will get way too easy and predictable, thus - boring.

Besides, it happens to everyone, so finding a good way to manage these situations could turn into an advantage for the good manager. ;)

My issue isn't with the 'randomness' of it, I was more annoyed at the situation the substitutions occurred. There is no reason to make a sub with 20 seconds left in a blow-out game, other than injury or foul out. In the other situation i explained, basketball rules (at least in England) are that if you are subbing in for someone shooting a free throw, you don't sub in for another player, you have to wait until the next opportunity to sub in.

Having subs mid-game (3rd quarter for example) could happen for many other realistic reasons (low stamina, needs a rest etc) which i could understand.
