Fatigue seems to have a significant effect on FT shooting -- poor stamina players who have played tons of minutes tend to miss more free throws late in games. Additionally, you need a sufficient number of FTs shot to be able to make reliable inferences about expected probability -- above 30 FTs shot is an absolute minimum, above 50 is recommended.
And of course, there's always the luck of the draw. Even the most improbable outcome can happen to someone.
